i too had long term shoulder pain ,i had physio ,didn't work
i had an MRI ,didn't show anything
ultrasound showed a small tear on the bicep muscle which goes right over the top of your shoulder and attaches at the back
so i pushed for an arthroscopy
i had the operation on th 3rd and they found that bicep attachment point mashed up
althought it only caused me pain when i did certain things like throwing ,driving or pulling myself up,or prolonged use with my arm in the air so they removed that attachment point from round the back to round the side
i am so glad it could be fixed and apart from not being able to use my bicep for 6 weeks and massive pain it should be ok in the future
get all the tests you can and if they still don't find anything go for exploratory surgery
GOOD LUCK

Your shoulder pain may have developed from a bad postural habit. Say for example, for the past few years, you almost always have your shoulder "elevated". An instance is when you use a table as high as your chest which in turn makes you "elevate" your shoulder to an uncomfortable level (if you use the table for a long time). Another common cause of shoulder pain (in women) is carrying heavy shoulder bags. It can greatly affect your shoulder as well. First, pain will be on the affected side then due to continued practice the other shoulder may "compensate" thus explains the "creeping" part you were talking about. These kind of habits affect your "TRAPEZIUS" muscle. This muscle is the one covering your shoulders. It assists your shoulder in lifting objects above shoulder level.

I'm not a professional but I hope it helps~
