They can x-ray you and make a diagnosis. I had some major shoulder pain due to pushing something very heavy and although I did lose some loss of motion, I was able through therapy to lessen the pain and add to the motion I lost. Long story short: you buy a an OVER-THE DOOR CERVICAL TRACTION KIT, at CVS , Walmart etc. the kit contains a big plastic bag, a rope, a harness for your chin and head, and a pulley that rests on top of a door you use it two-three times a day, 15 minutes per session, Only 15 minutes NOT any longer You fill the bag with water , it goes by your weight , I weigh 125 lbs so I fill the bag with 3 lbs of water my husband also has this problem he weighs 200 lbs so he uses 5 lbs of water pounds of water (there's a red line that indicates that amount as well as lesser volumes), put the harness under your chin and skull, sit down in a chair, and let the weight — via the rope and pulley — exert a steady pull upward under your head. That's it. Nothing will happen until about the fourth day, when you will suddenly noticed that the pain seemed less intense .. With each subsequent session it will get better. After another week or so the pain and ache will be gone ..
