I know this sounds like a stretch but pain in the left shoulder is often a symptom of heart issues (look up the symptoms of a heart attack). The only reason I throw this out there is because you mentioned your whole body tightening up which makes it a little suspicious. I mean, I may be wayyyy off here. I don't know what exactly you are feeling and it may just be a nerve issue but I felt the need to mention that because the consequences of missing something like that can be so serious. If you have some of the qualifiers for heart disease or something, I would have the doctor run some heart tests. Hopefully, it's just some quirky nerve thing.
